PAYMENT
Payment is due at the time of service. I accept all major credit cards, FSA account, Zelle, and PayPal.
INSURANCE
I am in-network with most major insurance plans, including Anthem, Aetna, Blue Cross Blue Shield, Medicare, and Medicaid. Not sure if your plan is covered? Reach out and I’ll check for you.
cancellation policy
I understand that plans can change unexpectedly. If you need to cancel or reschedule, please provide notice at least 24 hours in advance. Cancellations made less than 24 hours before your scheduled appointment may result in a $100 cancellation fee. The fee must be paid before another appointment is scheduled.
